29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system: On or about 8/15/18, during residential roofing activities, located at Hampton Place Ryan Homes Lot #143 in North Ridgeville, Ohio, the employer failed to ensure that employees were provided and utilized fall protection. The roof slope was 9/12 and the heights varying between 11 feet and 14.3 feet.
