29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: (a) At the construction site located near 2140 W. St. Paul Ave, Waukesha WI, employees were performing work in an approximately six feet deep excavation without adequate cave-in protection, thereby exposing employees to a struck-by / crushing hazard.
