29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminants chemical state and physical form. When the employer was unable to identify or reasonably estimate the employee exposure, the employer did not consider the atmosphere to be IDLH: Location - pour line On or about August 9, 2018, employees were exposed to splashed Toluene Diisocyanate due to fractured visual sight glass at the pour line deck. The employer had not performed an evaluation or identification of respiratory hazards for potential IDLH atmosphere or overexposure to chemical hazards such as, but not limited to, Toluene Diisocyanate.
