29 CFR 1910.132(a):  Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:                                                                                           The employer is failing ensure that employees are protected from the hazard of chemical burns. This most recently occurred at 940 Central St., Osceola, Nebraska 68651 on August 6, 2018.  An employee did not use splash goggles/facemask when performing a maintenance task on the ammonia storage tank pump.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.
