29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about August 15, 2018; roof area; employees were not wearing fall protection while removing roof shingles exposing them to an estimated fall hazard of 30-35-foot to ground level.  The EMJ Contractors Inc. was previously cited for a violation of this occupational safety and health standard  29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1064859, citation 1, item 1 and was affirmed as a final order on 9/9/2015, with respect to a workplace located at 3424 22nd Street NE.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) On or about August 15, 2018; roof area; the employer did not certify the fall protection training given to the employees that were exposed to an estimated 35-foot fall hazard.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
