29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into or stepping into or through any hole that was less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: A. On or about June 8, 2018, Shipping Bay: The gap between the dock and the trailer was not provided with a cover or guardrail to prevent employees from falling 3 feet 9 inches to the pavement below. The mounted bridge plate and portable dock plate were not put into place before employees entered the trailer. An employee sustained a laceration injury requiring stitches.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: A. On or about August 21, 2018, Warehouse, Shipping Area: A black and yellow flexible extension cord was used in lieu of the fixed wiring of the building. The extension cord, energized through a black 6-outlet relocatable power tap, was used to power a 43" diameter Max Air Pro metal-encased floor fan used for area cooling.
