29 CFR 1910.23(b)(12): Each employee did not use at least one hand to grasp the ladder when climbing up and down it.  Kitchen Fan Room, roof access-  The access hatch at the top of the fixed 12' ladder was secured with a rope. This required the user to untie a knot prior to climbing out through the hatch, making it difficult and hazardous to negotiate the roof access. The hatch would then need to be secure the same way when descending the ladder.

29 CFR 1910.23(d)(7): Grab bars did not extend 42 inches (1.1m) above the access level or landing platforms served by the ladder.   Kitchen Fan Room, roof access-  The access hatch at the top of the fixed 12' ladder does not provide a grab bar while ascending or descending the roof, exposing employees to 14' falls to the lower level from the roof.

29 CFR 1910.29(b)(2): The employer did not ensure that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high:  Cafeteria Skylights-  (a) The guardrail system had no midrails for any of the Cafeteria's main skylights and sixteen smaller ones, exposing employees to falls of approx. 24 feet.

29 CFR 1910.29(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds applied in a downward or outward direction within 2 inches of the top edge, at any point along the top rail.  Cafeteria Skylights guardrail systems-  (a) The top wire rope cable of the guardrail system was ineffective, as demonstrated when downward hand pressure was applied to the top rail cables which allowed them to deflect 20 to 35 inches.  As a result employees were exposed to serious injuries or death from falling approx. 24 feet.

29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors.   Central Plant-  (a) Employees were not able to open an exit route door without sliding a dead bolt and turning a door knob.

29 CFR 1910.303(g)(2)(i): 29 CFR 1910.303(g)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by approved cabinets or other forms of approved enclosures, or other means listed under this provision:  (a) The 110 volt receptacle located on the outside back wall of the shop building had exposed live terminal screws exposing employees to electrical shock.   The Bureau of Indian Affairs, Chemawa Indian School was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(g)(2)(i), which was contained in OSHA inspection number 1014816, citation number 1, item number 11 and was affirmed as a final order on 6/30/2015, with respect to a workplace located at Salem, Oregon 97305.

29 CFR 1910.157(e)(1): The employer shall be responsible for the inspection, maintenance and testing of all portable fire extinguishers in the workplace:   Central Plant-  The employer had contracted out the inspections of fire extinguishers, however some of them have been missed and monthly inspections had not been completed.
