29 CFR 1910.303(f)(2): Services, feeders, and branch circuits. Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident. a. On or about August 16, 2018, in the electrical room, each circuit breaker in the north service panel-A was not marked to indicate its purpose, thereby exposing employees to an electrical and/or fire hazard. b. On or about August 16, 2018, in the electrical room, each circuit breaker in the south service panel-A was not marked to indicate its purpose, thereby exposing employees to an electrical and/or fire hazard. c. On or about August 16, 2018, in the electrical room, the north disconnect was not marked to indicate its purpose, thereby exposing employees to an electrical and/or fire hazard. d. On or about August 16, 2018, in the electrical room, the disconnect for the compressor on the main switch cabinet was not legibly marked to indicate its purpose, thereby exposing employees to an electrical and/or fire hazard. e. On or about August 16, 2018, in the electrical room, the disconnect for the transformer feeds panel A and west wall on the main switch cabinet was misidentified as to its purpose, thereby exposing employees to an electrical and/or fire hazard.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed. a. On or about August 16, 2018, in the electrical room, the north service panel-A had four openings in the right side due to knockouts that were missing and/or conduit that was removed, thereby exposing employees to an electrical hazard. b. On or about August 16, 2018, in the electrical room, the north unidentified disconnect had an opening on the top side due to a knockout that was missing, thereby exposing employees to an electrical hazard.
