29 CFR 1926.501(b)(14): The employer did not ensure that each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: On or about August 16, 2018, and at times prior to, employees were exposed to falling 20 feet to the ground below when dumping debris out of windows and fall protection systems were not used.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: On or about August 16, 2018, employees were conducting demolition activities and were provided with filtering facepiece respirators and the employer had not provided basic advisory information to the employees.
