On or about August 16, 2018, at the above addressed jobsite, employees were exposed to slip, trip and fall hazards when the bottom of the ladder was not kept clear of debris, including but not limited to, piles of discarded shingles and nails. Additionally, the ladder that was used to access the ladder was the material hoisting ladder, with a elevator that sat at the bottom of the ladder approximately two (2) feet six (6) inches from the ground.
