29 CFR 1910.252(a)(2)(xiv)(C)(3): The employer did not ensure that the supervisor saw that cutting and welding was so scheduled that plant operations that might expose combustibles to ignition were not started during cutting or welding: On or about August 9, 2018, and in time prior thereto, the employer did not ensure that welded metal in the grain handling area was not cooled before allowing the unloading of grain in the hopper, exposing the welder to an explosion hazard.

29 CFR 1910.304(a)(2): Grounded conductors were attached to terminals or leads so as to reverse designated polarity: On or about August 16, 2018, and in times prior thereto, the outlet used to supply power to an electric arc welder in the grain handling area had the polarity reversed, leaving the welder energized while in the off position. Employees were exposed to an electrical hazard.
