29 CFR 1910.217(c)(3)(iv)(b): Attachments for pullout device(s) used on mechanical power press(es) were not adjusted to prevent the operator from reaching into the point of operation or from withdrawing his hands from the point of operation before the die. On or about 8/1/2018 in the Press Room, the employer did not ensure that a recently assigned employee was protected from amputation hazards created by the point for operation of the 56 ton mechanical power punch press (#563) by ensuring that hand pull-back safety devices (i.e. point of operation devices) continued to be utilized by the operator during operation of the press.

29 CFR 1910.217(f)(2): The employer did not ensure by adequate supervision that correct operating procedures were being followed: On or about 8/1/2018 in the Press Room, the employer did not ensure that a recently assigned employee was protected from amputation hazards created by the point of operation of the 56 ton mechanical power punch press (#563) by ensuring that the operator received adequate supervision to ensure that they were continuing to wear and correctly using hand pull-back safety devices (i.e. point of operation devices) while operating the press. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
