29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a): Exterior - The employer did not ensure that three employees were using appropriate eye protection while controlling a concrete pump used to pour concrete inside a concrete block wall, on or about August 16, 2018.  There is no abatement certification or documentation required for this item.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    (a): Interior Scaffold Access - Employees were exposed to fall hazards of approximately 3 feet while accessing a fabricated frame scaffold. The employees were accessing the scaffold platform which was approximately 3 feet in elevation, on or about August 16, 2018.  There is no abatement certification or documentation required for this item.
