29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about August 15, 2018 at 1453 Highway 120E, Lawrenceville, Georgia, five employees wee observing installing a new rubberized membrane on the roof. The employees were exposed to a fall hazard of approximately 13 feet to the lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about August 15, 2018 at 1453 Highway 120E, Lawrenceville, Georgia, five employees working on the roof installing a new elastic roof, used an extension ladder that did not extend three (3) feet above the landing for safe access/egress.
