29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about June 18, 2018 at 612 Indigo Court, Woodstock, Georgia:  a) an employee passing mud up to the worker on the upper level was not wearing a hard hat to protect him from falling debris.  b)  an employee engaged in masonry work was working beneath another employee while not wearing a hard hat to protect him from falling debris, tools, or other material.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used.  On or about June 18, 2018 at 612 Indigo Court, Woodstock, Georgia, an employee working with the electrical mixer was exposed to an electrical shock hazard when a proper splice/repair was not made to the mixer electrical cord.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights is no than 1 inch wide.  On or about June 18, 2018 at 612 Indigo Court, Woodstock, Georgia:   a) workers working from the second and third level of a tubular welded frame scaffolding installing stone on the side of the residence did not have each working level fully decked.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

On or about June 18, 2018 at 612 Indigo Court, Woodstock, Georgia, employees were on a tubular framed scaffold installing split brick on the outside of the residence. The scaffold legs did not rest on base plates, mud sills, or any other adequate support. 29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.    On or about June 18, 2018 at 612 Indigo Court, Woodstock, Georgia, when employees engaged in masonry work; a competent person was not available to direct and supervise the erection of the scaffolding.  The following discrepancies existed at the time of the inspection to include, but not limited to the following:    a.  the working levels were not fully planked.    b. a ladder was not provided of a safe means of access/egress.    c.  all scaffold legs did not rest on base plates, mud sill, or other adequate support.    d.  there were no guard rails or end rails.

29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold was not protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway.    On or about June 18, 2018 at 612 Indigo Court, Woodstock, Georgia, employees working on a tubular welded frame scaffold were exposed to a fall hazard of more than 10 feet to the lower level when the guard rails were not properly installed on each of the working levels.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
