29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a- Plaza Grande at Garden State Park, Building 700, Cherry Hill NJ: A company employee was exposed to fall hazards when working from an elevated man lift basket of an articulated boom lift without the use of the required fall protection. At the time of the exposure this employee was not wearing the required fall protection. Condition observed on or about 08/18/18.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a- Plaza Grande at Garden State Park, Building 700, Cherry Hill NJ: Company worksite employees were exposed to fall hazards in excess of 40 feet to the ground surface below when walking/working from a pitched roof of a commercial structure while performing roofing and other related activities while either not using any fall protection or while wearing a full body harness with no lanyard attached. Condition observed on or about 08/18/18. AF Home Improvement Corporation was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 946804, citation number 1, item number 2 and was affirmed as a final order on 02/19/14, with respect to a workplace located at Building 2/4 Conklin Lane, Raritan NJ.
