29 CFR 1926.502(d)(6)(iv): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself:  a) Floor Decking on 2nd, 3rd and 4th Floors - On or about August 15, 2018 and at times prior to, the employer did not ensure that anchorages were adequately installed for personal fall arrest systems and allowed employees to attach snap hooks directly to the horizontal lifeline while installing floor decking, thus exposing employees to fall hazards of up to approximately 30 feet.

29 CFR 1926.1052(b)(1): Foot traffic was not prohibited on stairways where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date and stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan:  a) Plan West, Stairway A - On or about August 15, 2018, the employer did not ensure that employees were prohibited from using the stairway with pan stairs that were not adequately filled with material, thus exposing employees to trip and fall hazards.
