On or about August 10, 2018, during the installation of roofing material at the job site located at 6423 S. Salem Warren Rd, North Jackson, Ohio 44451; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.
