29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. 29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area:  (a)  RC Roofing Denver Inc. located at 1294 Madison Ave. Loveland, CO 80537:  On and preceding 17 August 2018, The exposing employer did not ensure that warning lines were erected around all sides of the roof work area.  Roofers on a flat roof and a 2 foot parapet were applying TPO without a properly configured warning line system or safety monitor in place.  The waning line system was only erected around the inner portion of the roof and not around the outside portion of the roof where work was taking place. This exposed employees to falls from height.

Roofers on a flat roof and a 2 foot parapet were applying TPO without a properly configured warning line system or safety monitor in place.  The waning line system was erected 6-12" from the edge of the inner portion of the roof. This exposed employees to falls from height.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a)  RC Roofing Denver Inc. located at 1294 Madison Ave. Loveland, CO 80537:  On and preceding 17 August 2018, The exposing employer did not ensure that the portable ladder used for access to the roof extend at least 3 feet above the upper landing surface being accessed. Roofers on a flat roof were applying TPO using an extension ladder through a hatch going on to the roof which extended approximately 15" beyond the upper landing.  This exposed employees to falls from height.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a)  RC Roofing Denver Inc. located at 1294 Madison Ave. Loveland, CO 80537:  On and preceding 17 August 2018, The exposing employer did not develop, implement, and/or maintain at the workplace a written hazard communication program for roofers that used chemicals which include but are not limited to TPO bonding adhesive.  When requested, the foreman stated that there was no hazard communication program in place.

Five roofers were using chemicals which include but are not limited to bonding adhesive to apply TPO to the roofing surface.  The employer was not able to provide an SDS for the chemicals being utilized on the job site during the inspection.
