29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes, by covers:    (a)  On or about June 29, 2018, and at times prior, Gonzales Concrete Inc., as creating employer, did not ensure employees of Whiting-Turner Contracting Company, general contractor, and employees from multiple subcontracting companies working at the site, were protected from tripping in or stepping into floor holes that ranged from 8 to 17 inches deep, and 2 to 17 inches wide.  Approximately 38 floor holes were exposed on the floor of the building that was under construction where workers and employees were walking.    Abatement Note:  Abatement certification is required for this item (See Certification of Corrective Action Worksheet).
