29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  On or about 05/09/2019, the employer failed to certify to OSHA that the following hazards have been abated:  (a) Citation 1, Item 1 - Lot 62: On or about August 8, 2018 an employee was exposed to fall hazards while loading and installing exterior bricks on a townhome from an arched masonry scaffold 12 feet 8 Inches above ground level. The scaffolding had two 6 inch gaps between the wood planking and/or uprights.  (b) Citation 1, Item 2 - Lot 66, front of building, scaffold; On or about August 8, 2018 employees were exposed to fall hazards of up to 13 feet while installing bricks on a townhome from an arched masonry scaffold without a safe access installed.

29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  On or about 05/09/2019, the employer failed to provide documentation to OSHA demonstrating that the following hazards have been abated:  (a) Citation 1, Item 3a - Lot 66. Front of building, Scaffold; On or about August 8, 2018 three employees were exposed to falls of up to 13 feet while using an arched masonry scaffold to install bricks on a townhome without guardrails installed on the scaffolding.  (b) Citation 1, Item 3b - Lot 66, front of building, Scaffold: On or about August 8, 2018 employees were exposed to fall hazards of 13 feet while working from an arched masonry scaffold installing brick on a townhome with 10 foot long scaffold platforms extended over the supports by 2. 5 feet on the main deck and 3 feet in the outrigger area.
