The scaffolding had two 6 inch gaps between the wood planking and/or uprights. (b) Citation 1, Item 2 - Lot 66, front of building, scaffold; On or about August 8, 2018 employees were exposed to fall hazards of up to 13 feet while installing bricks on a townhome from an arched masonry scaffold without a safe access installed.

(b) Citation 1, Item 3b - Lot 66, front of building, Scaffold: On or about August 8, 2018 employees were exposed to fall hazards of 13 feet while working from an arched masonry scaffold installing brick on a townhome with 10 foot long scaffold platforms extended over the supports by 2. 5 feet on the main deck and 3 feet in the outrigger area.
