29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:   a) 645 Fretz Rd. Mainland, PA, Production Area - on or about August 17, 2018, a refuse conveyor system was installed into the shop floor, which resulted in a condition where the walking-working surface was affected by protruding objects created by the system and its limited metal guarding.

29 CFR 1910.147(e)(3): Each lockout or tagout device shall be removed from each energy isolating device by the employee who applied the device. Exception to paragraph (e)(3): When the authorized employee who applied the lockout or tagout device is not available to remove it, that device may be removed under the direction of the employer, provided that specific procedures and training for such removal have been developed, documented and incorporated into the employer's energy control program.  a) 645 Fretz Rd. Mainland, PA - on or about August 17, 2018, employees performed servicing and maintenance on machines to include, but not limited to, Viking nailing machines, notchers, gang saws, and porter saws, but the employer had provided only one lock for employees to share with multiple, accessible keys available for the control of hazardous energy sources.

R & R Wood Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.333(a), which was contained in OSHA inspection number 1233698, citation number 1, item number 4, and was affirmed as a final order on August 11, 2017, with respect to a workplace located at 645 Fretz Rd. in Mainland, PA.

29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  a) 645 Fretz Rd. Mainland, PA - on or about August 17, 2018, employees used chemicals to include but not limited to diesel fuel which had been transferred into portable containers for use that were not labeled with all required information to include hazard information, or pictures, symbols,, or combination thereof, which would have provided the necessary information to those employees utilizing the chemicals.  R & R Wood Products, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(f)(6)(ii), which was contained in OSHA inspection number 1233698, citation number 1, item number 5b, and was affirmed as a final order on August 11, 2017, with respect to a workplace located at 645 Fretz Rd. Mainland, PA.
