29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) On or about August 28, 2018 and times prior; wrap machine location #4, machine #17, perishable building: The employer failed to ensure an employee changing a roll of film properly locked out the electric and pneumatic power sources in that only the e-stops were used, which exposed him to the hazards associated with the inadvertent start-up of equipment.    b) On or about September 11, 2018 and times prior; wrap machine location #1, machines #2, #3, #3, and #4, dry building: The employer failed to ensure an employee sweeping both sides while within the robot arm movement area properly locked out the electric and pneumatic power sources in that only some e-stops were used, which exposed him to the hazards associated with the inadvertent start-up of equipment.    c) On or about September 12, 2018 and times prior; wrap machine location #4, machine #17, perishable building: The employer failed to ensure an employee changing rolls of film properly locked out the electric and pneumatic power sources in that only the e-stops were used, which exposed him to the hazards associated with the inadvertent start-up of equipment.    d) On or about September 13, 2018 and times prior; wrap machine location #2, machines #4, #1, #1, and #2, dry building: The employer failed to ensure an employee sweeping both sides while within the robot arm movement area properly locked out the electric and pneumatic power sources in that only some e-stops were used, which exposed him to the hazards associated with the inadvertent start-up of equipment.    e) On or about September 13, 2018 and times prior; wrap machine location #2, machines #4, #1, #1, and #2, dry building: The employer failed to ensure an employee changing rolls of film properly locked out the electric and pneumatic power sources in that only the e-stops were used, which exposed him to the hazards associated with the inadvertent start-up of equipment.

29 CFR 1910.178(m)(10): Load backrest extension(s) were not used on industrial trucks to minimize the hazard of rearward falling materials:    a) On or about September 1, 2018 and times prior; dry goods loading area: The employer failed to ensure a backrest extension was added to prevent material from falling toward the operator which exposes the operator to hazards associated with being struck in the head and/or upper body.    b) On or about July 7, 2018; dry goods loading area: The employer failed to ensure a backrest extension was added to prevent material from falling toward the operator which resulted in the operator being struck on the left hand and possibly head resulting in a lost time injury.    c) On or about August 13, 2018 and times prior; dry goods loading area: The employer failed to ensure a backrest extension was added to prevent material from falling toward the operator which exposes the operator to hazards associated with being struck in the head and/or upper body.
