29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating: A. On or about August 23, 2018, Hallway: The Hyster 25 propane-powered forklift, #105, was provided with two metal straps to hold the LPG tank in place. The strap on the right of the propane tank was broken and did hot secure the tank.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): A. On or about August 23, 2018, Facility: Employees were allowed to operate powered industrial vehicles, including the Hyster 25, Komatsu FG25S and Hyundai HLF15C-5. Operators had not been provided training and evaluation prior to being allowed to drive the equipment.
