29 CFR 1926.502(d)(6)(i): Snap hooks were connected directly to webbing, rope or wire rope: (a) At each of the HVAC duct floor openings on the 2nd, 3rd and 4th floors, on or about August 15, 2018 and at times prior to, the employer did not ensure that anchorages were adequately installed for personal fall arrest systems and allowed employees to attach snap hooks directly to webbing while they were installing HVAC duct work, exposing them to a fall hazard of up to approximately 30 feet.
