29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  (a) 1001 N 17th Street, Harrisburg Pennsylvania - The employer did not certify that the citations had been abated within 10 days of the abatement date, on or about December 13, 2018.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) 1001 Nth 17th Street - Employees conducting roofing operations on a 10 in 8 pitch roof were not attached to a fall arrest system and were exposed to hazards of approximately 30 feet to the lower level, on or about August 21, 2018.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.  Quality Exterior Remodeling, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1319754, citation 1 item 1 and was affirmed with a final order on August 10, 2018, with respect to a workplace located at 77 Grant Street, Manheim, Pa

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:  (a) 1001 Nth 17th Street - Employee was observed picking debris up from the ground while employees on the roof were throwing debris to the lower level without a protective helmet, on or about August 21, 2018.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
