29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about August 23, 2018 and at times prior to, employees removed jams from the chain and sprocket, removed contaminated product from the roller and deep clean various equipment including, but not limited to the Tenter #1.  Employees were exposed to struck by, caught in and amputation hazards when the employer did not perform a periodic inspection to verify that the procedures are adequate and being properly applied.

Employees were not able to identify and properly lockout all energy sources and were exposed to struck by, burns, caught in and amputation hazards.

29 CFR 1910.219(f)(1)(i): Gears, sprockets, and chains were not guarded by a complete enclosure.   a) On or about August 23, 2018 and at times prior to, employer did not ensure that all gears, sprockets, and chains were guarded by a complete enclosure. Employees were exposed to amputation and caught in between hazards when the guard from the Tenter #1, sprockets and chains were not fully enclosed leaving opening up to six inches in the guard.
