29 CFR 1926.20(b)(2) Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On or about 8/20/2018, a subcontractor of Musick Construction Company was welding structural steel on a commercial building located at 2000 West Clay in St. Charles, MO. Musick Construction Company did not conduct an adequate inspection of the job site, materials, and equipment. During the inspection, an employee was welding the steel structure while working from a Genie Aerial Lift, Model S-45/Serial #1000628. He was exposed to the following hazards: a) The employee was not standing on the floor of the basket. The employee was observed climbing the guardrails and welding while standing on the mid and top rails of the basket. b) The employee was not wearing body belt with a lanyard or personal fall arrest system that was attached to the boom or basket while working from the aerial lift.
