On or about August 20, 2018, employees were exposed to a fall of approximately 10 feet while performing roofing activities on a residential home located at 330 Grand, Bedford, Ohio 44146. The employees were working on all areas of the roof and the employer did not ensure that they were trained on how to recognize fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about August 20, 2018, employees were exposed to a fall of approximately 10 feet while performing roofing activities on a residential home located at 330 Grand, Bedford, Ohio 44146. The employees were using multiple extension ladders to access the roof and the employer did not ensure that they extended three feet above the roof landing area.
