29 CFR 1910.176(f): Derail and/or bumper blocks were not provided on spur railroad tracks where a rolling car could contact other cars being worked or enter a building, work or traffic area: SITE YARD: The employer had not derailed and/or provided bumper blocks on the rail tracks where the three (3) rail cars being worked could come in contact with the eight (8) outbound rail cars. On/or about August 16, 2018, the coupler was not connecting appropriately, and the three (3) rail cars came in contact with the eight (8) rail cars while an employee was in the process of lining up the three (3) rail cars with the eight (8) rail cars. The employee sustained broken bones to the arm from the incident.
