This condition exposed employees to fall hazards.

29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were intermixed such that the components fit together with force and the scaffold's structural integrity was not maintained by the user: (a)Ahoe Construction at 415 South 8th Street, Aberdeen, SD 57401: On or about and at times prior to August 20, 2018, the exposing employer did not ensure the structural integrity of the scaffolding system was maintained in that, the employer intermixed scaffold components when using a ladder jack to support an extension ladder between two frame scaffolds to be used as a work surface. This condition exposed employees to fall hazards greater than 6 feet above ground level.

The scaffolding systems did not bear on base plates, mud sills or other adequate firm foundations. The end frames were erected without the use of baseplates and were resting directly on 2x4's and 2x10 material. This condition exposed employees to fall hazards.

This condition exposed employees to fall hazards. (b)Ahoe Construction at 415 South 8th Street, Aberdeen, SD 57401: On or about and at times prior to August 20, 2018, the exposing employer did not ensure employees were protected from fall hazards in that, an A-frame ladder was leaned against a frame scaffold to provide access without the ladder spreaders being opened and locked. This condition exposed employees to fall hazards. (c)Ahoe Construction at 415 South 8th Street, Aberdeen, SD 57401: On or about and at times prior to August 20, 2018, the exposing employer did not ensure employees were protected from fall hazards in that Keller KMT-13 type 1A ladder was used in conjunction with a ladder jack. This condition exposed employees to fall hazards.

This condition exposed employees to fall hazards between 11 to 23 feet above ground level.
