On 8/21/2018, CCIMW, LLC was painting the siding and soffits on a building located at 5856 Suemandy Drive, in St. Peters, MO. An employee was utilizing a JLG Aerial Lift, Model 340 AJ/Serial # 0300197500, to access the roof of the building. The employee was not wearing a personal fall arrest or travel restraint system that was attached to the boom or basket while utilizing the aerial lift.

On 8/21/2018, CCIMW, LLC was painting the siding and soffits on a building located at 5856 Suemandy Drive, in St. Peters, MO.
