29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a). On or about 8/6/18, on site, for employee mixing paints using power strip to operate scales, mixers and compressor. A Relocatable Power Tap (power strip) was used to provide power to various scales, mixers, an air compressor, and a fan. An extension cord was connected to the power strip that led to an air compressor used on site. The power strip is not intended to be used for high powered equipment nor to be connected to other extension cords.

On or about 8/6/18, on site, for employees mixing paints for autobody shops. The employees are exposed daily to UR50 Reducer, which is used for all mixes. UR50 Reducer contains chemicals to include, but not limited to, n-Butyl acetate and 2-butoxyethyl acetate. The employer did not implement a hazard communication program to include a written program and training.
