29 CFR 1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:  (a) 474 Valor Drive, Mechanicsburg Pennsylvania - The employer did not certify that the citations had been abated within 10 days of the abatement date, on or about October 10, 2018.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a)  474 Valor Drive Mechanicsburg, PA - Employees were performing roofing activity on a steep residential roof, with an 8 in 12 pitch without the benefit of a complete fall protection system, thereby exposing employees to a fall of 11 feet 6 inches to the ground below, on or about August 3 12, 2018.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) 474 Valor Drive Mechanicsburg, PA - The employees were using an aluminum extension ladder leaning up against the front wall to access the residential roof and did not extend three feet above the landing, exposing the employees to injuries from ladder collapse or deflection, on or about August 3, 2018.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement. .
