The employer failed to remove the powered industrial truck from service until the repairs could be made.

On or about 8/21/2018, in the lumber storage area at 331 Thomson Highway, Warrenton, GA: The employer failed to report to OSHA a work-related hospitalization and amputation within 24 hours of such knowledge and did not contact OSHA of the following injury: 8/11/2018, Michelle Norman, the Powered Industrial Truck Operator was hospitalized with a fracture to her left knee. OSHA became aware of this hospitalized employee on 8/21/2018.
