29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Facility Wide- On August 21, 2018, the employer failed to develop machine specific procedures for the control of hazardous energy sources such as electrical, hydraulic and gravitational energy for machines including the Cincinnati(1) and Amada(3) press brakes. Maintenance employee performed lockout when conducting servicing and maintenance activities on machines. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase, or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolate (a) Production - On August 21, 2018, the employer did not ensure that press brake operators, performing die setting, were provided training in the recognition of applicable hazardous energy sources such as electrical, hydraulic and gravitational energy for the Cincinnati(1) and Amada(3) press brakes and the methods and means necessary for energy isolation. In accordance with the requirements of 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete and must be included with your certification. 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): 1.Turn off and shut down power to the machinery, 2.Physically locate and operate energy-isolating devices in such a manner as to isolate the machine or equipment from the energy source(s), 3. Affix lockout/tagout devices to each energy isolating device, 4. Verify isolation and de-energization of machinery have been accomplished prior to starting the maintenance and/or servicing work: a) Production Department- On August 21, 2018, machine operators were exposed to machine hazards associated with moving parts while performing die setting on press brakes. The employer did not implement energy control application steps as the machine was not shut down or turned off to perform die setting [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving dissipation machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.
