29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At the Pooler jobsite: On or about August 15, 2018, and at times prior thereto, employees were exposed to fall hazards of 20 ft., in that (2) portable-ladder side rails were not extended at least 3 ft. above the upper-landing surface as workers used the ladders to gain roof-access.
