29 CFR 1910.134(c)(1):   A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  The employer is failing to protect employees from respiratory hazards.  This was most recently documented on August 22, 2018 at 4131 S. 89th St, Omaha Nebraska.  The employer is failing to create and maintain a written respiratory program as required by this sub part.  Employees are being exposed to concrete dust containing silica when cutting concrete with a Stihl TS420.

This was most recently documented on August 22, 2018 at 4131 S. 89th St, Omaha, Nebraska.  The employer is failing to ensure that the employees have been medically approved by a physician or other licensed health care provider to successfully wear respirators.  Employees were dry cutting concrete without the use of a respirator.

This was most recently documented on August 22, 2018 at 4131 S. 89th Street, Omaha Nebraska.  The employer is failing to ensure that employees have been able to achieve a tight-fitting respirator condition prior to the employees wearing respirators and performing work that would subject them to silica dust hazards.
