29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:  (a) On or about August 7, 2018, the employer did not ensure that employees were trained on how to properly use personal fall arrest systems in that an employee was not tied-off from his harness' D-ring but instead of a ring located closer to his pelvis. 29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      (a) On or about August 7, 2018, the employer did not ensure that employees using an extension ladder to access the roof of a multi-family residential home, was extended 3 feet above the landing surface.
