On or about 8/22/2018 an employer did not properly ensure that the work place was kept clean in that combustible wood dust was located throughout the facility such as but not limited to the 1st floor next to the belt sander, the routers and saws.  Wood dust had collected on electrical components located on the first floor such as but not limited to the electrical supply box for the lighting system, main shutoff for the belt sander and the junction box located next to the sprinkler drain off valve on the south wall.

The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    a) On or about 8/22/2018 employees who performed maintenance on belt sanders, band saws, table saws, routers, etc in the facility were not required to:    (1) Turn off and shut down the belt sanders, band saws, table saws, routers etc.   (2) Physically locate and operate energy isolating devices in such a manner as to isolate machine or equipment from the energy source.  (3) Affix lockout or tagout devices to energy isolating electrical device(s); and  (4) verify that isolation and de-energization of the machine or equipment have been accomplished.

Compressed air shall not be used for cleaning purposes except where reduced to less than 30 p.s.i. and then only with effective chip guarding and personal protective equipment.    a)On or about 8/22/2018 an employer did not properly reduce the PSI for the compressed air utilized by employees for the cleaning of themselves or equipment exposing employees to hazards associated with compressed air measured at greater than 30 PSI:     Test location #1 was in the middle of the facility in building #1, the readings were reading between 60 and 80 PSI, the gauge connected to the hose was reading approx. 70 PSI.     Test location #2 was located in Building #2, the readings were between 70 and 120 PSI, The gauge connected to the hose was reading 80 PSI.

The facilities roof was leaking in such areas as but not limited to the first floor, builder assembly area, above the dust extractors next to the south wall of the facility and the north wall ceiling next to the Beiss routers, allowing water to come in through the ceiling tiles onto the live electrical components such as main circuit breakers, power sources, junction boxes and control panels exposing employees to electrocution hazards.
