29 CFR 1904.29(b)(2): The employer did not complete an OSHA 301 Incident Report form, or an equivalent form, for each recordable injury or illness entered on the OSHA 300 Log: (a) On or about August 22, 2018 the employer's version of a 301 form did not include the detail required by the regulation for case number 180170007 that occurred on August 9, 2018. The form for this case did not include the full name of the person filling out the form, the title of the person filling out the form, the phone number of the person filling out the form, the date the form was filled out, the name of the healthcare professional treating the injured employee, the name and address of the medical facility where the employee received treatment, whether the employee was treated in an emergency room, and if the employee was hospitalized overnight as an in-patient.
