29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) A-1 International/Edison, NJ- Warehouse Area: The employer did not develop a written hazard communication program when employees were working with hazardous chemicals such as, but not limited to, propane. All provisions of 29 CFR 1910.1200(e) through (h) must be covered in a written hazard communication program. Key elements include, but are not limited to the following: 1) Chemical inventory list 2) Safety Data Sheets (SDS) 3) Labeling of containers 4) Employee information and training  Condition observed on or about August 22, 2018.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) A-1 International/Edison, NJ- Warehouse Area: The employer did not provide employees working in the warehouse with effective information and training on the hazardous chemicals used in their work areas at the time of their initial assignment or whenever a new hazardous chemical was introduced to the work area. Employees were operating propane powered forklifts exposing them to propane and carbon monoxide.  Condition observed on or about August 22, 2018.

Employees are operating propane powered forklifts and are required to change propane when empty.  Condition observed on or about August 22, 2018.

Injuries from Edison location were combined with those from other locations including Facebook, NY and Lyndhurst, NJ.  Condition observed on or about August 22, 2018.
