29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On or about August 23, 2018, employees were exposed to the hazard of falling approximately 40 feet while walking/working along an unprotected edge of the main roof of the building. b) On or about August 23, 2018, employees were exposed to the hazard of falling approximately 25 feet from a catwalk/lower roof being used as an access point to the main roof of the building. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
