29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59)  (a) 1340 Reed St., Philadelphia, PA 19147 - Employees were exposed to chemical hazards in that a written hazard communication program was not developed or implemented in the workplace where employees used hazardous chemicals, including:  - Tricalcium silicate, dicalcium silicate, tricalcium aluminate, tetracalcium aluminoferrite, and limesstone contained in Keystone Rainbow Bond Colored Masonry Cement.  This condition was observed on August 20, 2018.

(a) 1340 Reed St., Philadelphia, PA 19147- The employer did not provide safety data sheets for each hazardous chemical in the workplace, thus exposing employees to chemicals, including, but not limited to:  - Tricalcium silicate, dicalcium silicate, tricalcium aluminate, tetracalcium aluminoferrite, and limestone contained in Keystone Rainbow Bond Colored Masonry Cement.  This condition was observed on August 20, 2018.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)   (a) 1340 Reed St., Philadelphia, PA 19147 - Employees were using hazardous chemicals on a construction site, but the employer had not provided them with hazard communication training, thus exposing employees to hazards of chemicals.  Hazardous chemicals used on site included:  - Tricalcium silicate, dicalcium silicate, tricalcium aluminate, tetracalcium aluminoferrite, and limestone contained in Keystone Rainbow Bond Colored Masonry Cement.  This condition was observed on August 20, 2018.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  (a) 1340 Reed St., Philadelphia, PA 19147 - The employer did not provide personal fall arrest systems or guardrail systems where employees worked on the working level of a tubular welded frame scaffold approximately 32.5 feet above the lower level.  Employees were thus exposed to fall from height hazards.  This condition was noted on August 20, 2018.

29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:  (a) 1340 Reed St., Philadelphia, PA 19147 - The employer did not ensure that all frames of a tubular welded frame scaffold were braced while employees worked on the fifth level at a height of approximately 32.5 feet above the lower level.  Employees were thus exposed to fall from height hazards.  This condition was noted on August 20, 2018.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall:  a) 1340 Reed St., Philadelphia, PA 19147 - The employer did not ensure that toeboards or other devices to contain or defect falling objects while employees performed brick masonry on a scaffold and others employees worked on the ground below, thus exposing employees to struck-by hazards.  This condition was observed on August 20, 2018.
