Charles St., Superior, CO 80027: On or before August 8, 2018, the employer did not provide safety glasses to an employee that was using a pneumatic nail gun to lay shingles on a roof.

CFR 1926.501(b) (13): Each employee engaged residential construction activities 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems; (a) Juan Rios, 702 W. Charles St., Superior, CO 80027: On and before August 8, 2018, one employee working on 6:12 pitch, multi-level, single family homes roof was not protected from falling by a system as described above. Specifically, the employee was witnessed and photographed attaching shingles to the roof. Condition exposed employees to fall hazard of approximately 10 feet.
