29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a) On or about August 23, 2018 and at times prior thereto employees were accessing a roof 24 feet high to perform maintenance and troubleshooting and were not wearing fall protection.  NOTE-Abatement certification and documentation are required for this item.

29 CFR 1926.405(j)(1)(i): Fixtures, lampholders, lamps, rosettes, or receptacles had live parts normally exposed to employee contact:  (a) On or about August 23, 2018 and at times prior thereto employees were exposed to electrical hazards when internal wiring was exposed at a 110v outlet and a light switch, which had no cover plates, while power was applied.
