29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    On or about August 22, 2018, employees of Bonduelle were exposed to ingoing nip-points on machines that were not properly guarded. The areas where guarding was not adequate included:        a. Ingoing nip-points created a by Cobb Saw Chain Return on the Cobb Cutting Deck.      b. Ingoing nip-points created by moving conveyors in the Cobb Saw area on the Cobb Cutting Deck.

29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves.  On or about August 22, 2018, employees of Bonduelle working:      a. In the Repack Building on the Cob Corn Wrapping Line were exposed to rotating shaft ends which had a protruding Allen Screw.     b. In the Cobb Cutting Area were exposed to the rotating shaft and exposed bolts on the lower wheel section of the CCM Cutters.

29 CFR 1910.219(c)(4)(ii): Unused keyways shall be filled up or covered.  On or about August 22, 2018, employees of Bonduelle working in the Repack Building on the Cob Corn Wrapping Line were exposed to a rotating shaft end which had a keyway that was not filled in or covered.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment.  On or about August 22, 2018, Bonduelle did not ensure that electrical boxes were effectively closed to protect the wiring in boxes located:              a.  Above the gas alarms in the Ammonia Room.          b.  Above the conveyor of the west husker in the northwest portion of the outdoor corn receiving/processing area.

29 CFR 1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used: As a substitute for the fixed wiring of a structure;  On or about August 22, 2018, Bonduelle was utilizing a flexible cord in place of fixed wiring to connect Panel AA to Panel C3-1 on the Electrical Mezzanine in the Repack Building.
