29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around them:  Riverdale Avenue, West Springfield, MA: On or about August 23, 2018 employees were observed working around and near floor holes such as but not limited to an in ground concrete pool, first floor elevator maintenance shaft and HVAC chase way which were more than 6 feet above the next lower level and were not protected from falling through or stepping into them by means of fall arrest systems, covers or guardrail systems.  (a) In ground pool with fall heights up to 60 inches was not covered to prevent tripping into or stepping into. (b) Elevator shaft with no guard rails systems to prevent tripping or stepping into or through. (c) Mechanical duct work chase way with no guard rails to prevent employees from stepping into or through .

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  885 Riverdale Avenue, West Springfield, MA: On or about August 23, 2018 the employer had not verified compliance with paragraph (a) of this section by preparing a written record including the name or other identity of the employee trained, the date of the training, and the signature of the person conducting the training for employees exposed to fall hazards.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
