29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a) On or about 08/22/2018 at 715 Camarague Pl. Lake Mary, FL 32746 employees were exposed to a 19.4 foot fall hazard, in that, fall protection was not used. Employees were installing water proofing sheets, fascia siding and shingles on a 6/12 pitched roof.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    a) On or about 08/22/2018, at the job site - the rails of the ladder used to access the second floor and the top roof of the building did not extend at least 3 foot above the upper landing surface, exposing the employees to about a 19 feet fall hazard.
