29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) At the jobsite: Employees were not protected from falling while replacing a 12:12 pitched commercial roof at a height of approximately 40 feet from the top of the roof to ground level, as observed on or about August 23, 2018.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) At the jobsite:  Employees were performing commercial construction activities from a ladder jack scaffold and were not protected by a personal fall arrest system, exposing employees to a 30 foot fall hazard, as observed on or about August 23, 2018. 29 CFR 1910.1200(h)(3)(iv): Employee training did not include the details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information (Construction reference 29 CFR 1926.59):   a) At the jobsite: Employees worked with hazardous chemicals, such as the Low-VOC Bonding Adhesive UN1133, and the employer did not provide these employees with the required training on the new label elements and safety data sheet format as required by the revised OSHA Hazard Communication Standard incorporating the Globally Harmonized System of Classification and Labeling of Chemicals prior to the effective date of December 1, 2013, as observed on or about August 23, 2018.
